I have a form that uses dll's that had one AFFILIATE id and now uses another. not all ID's have been updated. Though there maybe another issue. it is working now under another domain. I have already update the domain referencs and the reports are functioning and the pages are all athenticating. Once the site is functioning we need to set commission 9 under 99 and 12 over 99 product sold.

I have a single form(edit.html.erb) that is written in Rails, and need it converted to PHP. The form is not complex, but populates some SELECTs from different tables and also stores data in several tables. Currently each input on the form saves data individually using Ajax. I want to simplify the form to use a single submit button. - form selects store name from table by looking up request.remote_addr - form then populates 5 select dropdowns with employee names - table of sales terminals is generated based on number of terminals in selected store (1 to 4 terminals) I have attached some images of the form, as well as form HTML from Rails version. Also attached is SQL of DB structure Some of the fields on the form calculate totals, but I will do that later if it make the project go more quickly

I have a drupal form and want to make one of the input boxes only display if "other" is selected from one of the <select> fields above it. Bonus points for a fading in background to give the user some visual indication that a new field is present. Should use jquery. Will be operating within a lightbox/modal window.

I am looking for someone with strong Ajax + php to help me with 1 page. I am using Xzero script for my classified site. Currently I am using ajax to populate countries, cities and areas to 3 drop down boxes and requires users to select which countries, cities (states ) and areas to post ads. Areas are optional fields. Users can select or not. I also have categories and sub-categories to be populated. My problem is: I can't submit the form and make it writes correctly into my database. I have a form on our site. Data in CSV files are to be entered into the form. There are about 4 fields to be filled (copy and paste). There are about 2000 +- 100 entries in each file. You need to understand English language as there is a field in the form with categories, and you have to select the best category that fits what you just entered. Bill per file. Make your best offer as there are several of these files to be processed.

Need a SILENT form submitter to post data on another website in the background. When a user clicks on a picture or video dialogue, i want the form in the background to be submitted. All information will come from databases on the main site. The other websites form will be iframed but will be covered by the pic/vid dialogue. Using PHP/Java/Curl/Ajax - the script should gather the next line in the database and enter the information on the form. When the user clicks anywhere they will actually be clicking submit on the background form. None of this will be seen by the end user. Each unique surfer will submit only one form and all entries will be a unique record from the database. Script needs to be functional in all browsers.

ASP form management (create, store data gathered, etc.) I need and ASP app that: 1. Can easely create forms/surveys and store the layout of them in a database for later publishing (drag and drop controls to put fields and easy design) DESIGN PHASE 2. Any user can ask for a specific form create it in step 1, the app should show the layoout of the form and the user fill it up, data will be store it in a data base. FILL IT IN PHASE 3. Every form filled can be subjet to an aproval or other actions.

Phase 1 1. Using Microsoft 2010 Excel, replicate a ten page hard copy set of State sales tax forms. Two of the hard copy pages are instructional and have no calculations, but need to be viewable in the same relative location as shown on the hard copy forms. 2. This spreadsheet must look exactly like the official sales tax forms. 3. The Excel system must allow the entry of all tax data in cells that represent the fields on the hard copy forms. There is no requirement (in this Phase 1) to create formulas to perform calculations. 4. All text areas of the spreadsheet must be protected so that I do not accidentally alter the form. 5. I have identified one company that has a website showing a similar product. Developer will create a tutorial explaining how the spreadsheet was created and how I can make minor modifications. Phase 2 1. Develop a front end system that allows for the entry, deletion, and edit of detailed sales data. This front end system should be a Microsoft 2010 Access system with the appropriate interface of data to the Excel Spreadsheet. This involves understanding the various tables of tax values and sales tax locations related to South Carolina zip codes. 2. The appropriate totals and detail created in the front end system will then populate the data fields in the Excel spreadsheet developed in Phase 1. 3. The spreadsheet created in Phase 1 will now require the development of the formulas to complete the automation of my sales tax calculations and subsequent printing of the completed State forms.
